Since the introduction of effective immunization, starting in the 1920s, diphtheria rates have dropped dramatically in the United States and other countries that vaccinate widely. Between 2004 and 2008, no cases of diphtheria were recorded in the United States. However, the disease continues to play a role globally. In 2007, 4,190 cases of diphtheria were reported, which is likely an underestimate of the actual number of cases.

ACIP amplia las indicaciones para la vacuna neumocócica conjugada 13-valente

Niños y adolescentes de 6 a 18 años con condiciones de alto riesgo para enfermedad neumocócica grave, deben recibir la vacuna antineumocócica conjugada 13-valente, aún cuando hayan recibido previamente la vacuna conjugada heptavalente.

ACIP toughens recommendations for PCV13

Some children and adolescents with certain high-risk conditions should receive the 13-valent pneumococcal conjugate vaccine, according to a recommendation from the CDC’s Advisory Committee on Immunization Practices.

Patients aged 6 to 18 years who require immunosuppressive medication, are immunocompromised, have cochlear implants, asplenia, HIV, cerebrospinal fluid leaks, chronic renal failure and nephrotic syndrome should receive PCV13 (Prevnar 13, Pfizer), regardless of having previously received 7-valent pneumococcal conjugate vaccine (Prevnar, Pfizer), according to the committee.

The committee’s recommendation followed a presentation on the vaccine’s use that was given by Tamara Pilishvili, MPH, of the Respiratory Diseases Branch, National Center for Immunizations and Respiratory Diseases at CDC.

Pilishvili said although the population of children who have these higher-risk conditions is small, they are at increased risk for severe disease. She added that the recommendation for the stronger wording harmonizes the ACIP recommendations with those recommendations for high-risk adults.

Changing What They Watch on the Screen Can Improve Children's Behavior

As most parents of preschoolers have witnessed, it’s common for children this age to imitate behaviors they see on television or in movies—whether violent, loving or something in between. This effect of media can be applied to positively impact children’s behavior, according to a study in the March 2013 issue of Pediatrics (published online Feb. 18). For the study, “Modifying Media Content for Preschool Children: A Randomized Controlled Trial,” researchers studied 820 families with children aged 3 to 5 years who spent some time viewing screens each week. Through their community pediatric practices, half of the families participated in an intervention in which they replaced aggression-filled programming with “prosocial” and/or educational content for the children. The other half of the families were in the control group. The intervention did not attempt to reduce the number of hours of screen time for the children, but it did encourage a positive media diet and co-viewing with parents. A case manager followed up with families regularly for 12 months. At 6 months and 12 months, the children in the intervention group were spending significantly less time on violent programming than they did at the start of the study compared to the control group. Both the intervention and control groups increased their viewing time slightly during the study, but the control group increased its minutes of violent content, while the intervention group increased its minutes of prosocial and educational content. At 6 months, the children in the intervention group demonstrated significantly less aggression and more prosocial behavior compared to the control group, and the effect lasted throughout the 12 months. The authors concluded that such an intervention can positively impact child behavior.

Synthetic Marijuana Linked to Acute Kidney Injury

Acute kidney injury (AKI) has been added to the growing list of deleterious health effects that include psychosis, birth defects, behavioral changes, and acute coronary syndrome associated with use of synthetic marijuana. 

A case series study conducted by investigators at the University of Alabama at Birmingham of 4 previously healthy young men linked AKI to ingestion of synthetic marijuana. "Cases of acute coronary syndrome associated with synthetic marijuana use have been reported, but our publication is the first to associate use with acute kidney injury," study coauthor Gaurav Jain, MD, said in a release. 

With first author Denyse Thornley-Brown, MD, the study will be published in the March issue of the Clinical Journal of the American Society of Nephrology. Also known as Spice, Spice Gold, and K2, synthetic cannabinoids have been marketed as various herbal mixtures and sold as incense, bath additives, and air fresheners. 

The investigators also note that there has been a "dramatic increase" in the use of synthetic marijuana preparations in the past 5 years. The relatively low cost ($20 per gram), coupled with the fact that these preparations are difficult to detect in routine drug tests, are cited as the main reasons for their growing popularity. 

They add that in the first half of 2010, there were 567 cases of synthetic cannabinoid use reported by US poison control centers in 41 states. In contrast, there were a total of 13 calls in the whole of 2009. The authors also note that despite laws in some 40 states against the use of these preparations, their use is increasing. "This is partly because new synthetic preparations that are not covered by legislation continue to be made and are available on the streets sometimes days after the legislation against prior preparations is introduced," the authors write. 

Synthetic marijuana preparations are typically smoked, although the researchers note there are reports of them being inhaled and ingested. "Well-recognized ill effects of these preparations of synthetic cannabinoids include impaired cognition in the immediate period after use, and long-term effects of dependence and withdrawal," the authors note. 

They add that 3 cases of acute coronary syndrome associated with Spice use were reported in young people. In their article, the investigators outline 4 different cases of AKI linked to ingestion of synthetic marijuana. All patients were members of the same community and presented over a 9-week period with symptoms of nausea, vomiting, and abdominal pain. Three of the patients had AKI marked by the excretion of an abnormally small volume of urine, and the fourth had a decrease in blood flow to the kidney. Three of the patients underwent a kidney biopsy that showed acute tubular necrosis. Left untreated, this can cause the kidneys to shut down. All 4 patients experienced recovery of their kidney function, and none required dialysis. 

Dr. Jain noted that given that these preparations often include several additives, it is likely that the causative agent in the AKI cases was an additive rather than the cannabinoid itself. "It is important to include nephrotoxicity from designer drugs such as Spice and bath salts in the differential diagnosis of AKI, especially in young patients with negative urine drug screens. Increased awareness may ultimately lead to identifications of the nephrotoxin(s)," the authors write.

Semana Mundial de la Lactancia Materna 2013

El lema de la Semana Mundial de Lactancia Materna de este año: Apoyo a las madres que amamantan: ¡cercano, continuo y oportuno! señala la importancia de la Consejería de Pares. Aún cuando las madres tienen un buen comienzo en su experiencia de amamantar, muchas veces, semanas o algunos meses después del parto, dejan de amamantar y bajan las tasas de lactancia materna. Cuando las madres no están frecuentando los centros de salud, es un momento clave para que funcione el sistema de apoyo comunitario. El soporte para mantener la lactancia puede brindarse de varias maneras. Tradicionalmente, la familia da un gran apoyo. Pero, conforme cambian las sociedades, en particular con la creciente urbanización, se necesita un círculo ampliado de apoyo, que puede estar integrado por personal de salud capacitado, consejer@s en lactancia, líderes de la comunidad o amistades que son madres también y por los padres o compañer@s.

La Consejería de Pares muestra un costo/beneficio positivo y es una manera productiva de llegar a muchas mujeres de manera más frecuente. Puede ser brindada por cualquier persona de la comunidad que esté capacitada en brindar apoyo a las mamás. Estas personas entrenadas y disponibles abren canales de comunicación para canalizar las preguntas y temas de lactancia que las mamás quieran plantear.
“La clave para una buena práctica de amamantamiento es tener apoyo cotidiano permanente en la casa y comunidad”.

Apoyo a las madres que amamantan:
¡cercano, continuo y oportuno!

CÍRCULOS DE APOYO PARA LAS MADRES, NIÑOS Y NIÑAS 
Los cinco círculos de apoyo a la lactancia materna ilustran el soporte a la decisión de las mamás de amamantar y al hecho de que tengan una experiencia positiva. En la Semana Mundial de Lactancia Materna del 2008, ya se había hablado de estos círculos de apoyo como algo vital para las madres: la familia y la red de apoyo social, el sistema de salud, los sitios de trabajo y empleo, los gobiernos y legislación, y las respuestas a las crisis o emergencias; todos ellos rodeando a las madres.

LAS MUJERES EN EL CÍRCULO INTERIOR
Las mujeres son el centro porque la presencia o ausencia del apoyo les impacta directamente. Las mujeres también juegan un rol central para asegurar el apoyo y para brindarlo a otras personas. La Iniciativa Mundial de Apoyo a las Madres, en su Declaración sobre Lactancia Materna del 2007, planteó: ¡Las madres son participantes activas en el apoyo dinámico, siendo tanto proveedoras como receptoras de información y apoyo”. FAMILIAS Y APOYO SOCIAL
Compañeros / padres / esposos, familia y amistades cercanas constituyen la red de apoyo más inmediata para las madres. El apoyo social incluye a la comunidad – en mercados, sitios públicos, contextos religiosos, parques vecinales, etc. El apoyo durante el embarazo reduce las tensiones. El soporte durante el parto y nacimiento empodera a las mujeres. El apoyo de la sociedad aumenta la confianza de las mamás en sí mismas y en su capacidad de amamantar más allá de las primeras semanas o meses.

SISTEMAS DE SALUD
Estos incluyen una multitud de oportunidades para apoyar la lactancia materna: cuido prenatal amigable, acompañamiento durante el parto y nacimiento, servicios post parto y de cuido post natal, todos ellos para apoyar el apego y la alimentación óptima. El personal de salud capacitado en habilidades de consejería apoya a las mamás antes y después del parto.

SITIOS DE TRABAJO Y EMPLEO
Las mujeres con empleo enfrentan retos particulares y necesitan apoyo para poder trabajar y amamantar a la vez. Las oportunidades de apoyo dependen de las formas de trabajo pero usualmente involucran facilitar el contacto madre-bebé, para extraerse y almacenar la leche materna.

GOBIERNOS/LEGISLACIÓN
Las mujeres que planean amamantar o que ya lo están haciendo, pueden beneficiarse de los documentos internacionales que protegen la alimentación infantil óptima, y de más y mejor financiados instrumentos como las comisiones nacionales. La legislación que protege contra el agresivo mercadeo de sucedáneos de la leche materna y las licencias pagadas por maternidad también son beneficiosas.

RESPUESTAS A CRISIS O EMERGENCIAS
Este círculo de apoyo representa la necesidad de apoyo cuando las mujeres se encuentran en una situación inesperada o muy seria que limita su control. Estos momentos requieren planificación y apoyo especial: desastres naturales, campos de refugiad@s, procedimientos de divorcio, enfermedad crítica de la madre o su bebé o cuando se vive en un área de alta prevalencia de VIH/SIDA sin apoyo a la lactancia.

Investigational TB vaccine lacks efficacy in infants

Vacunación tardía y esquemas incompletos de vacunación se constituyen en factores de riesgo para los niños, en los brotes de tos-ferina. Sin embargo, el uso de vacuna con componente acelular de B. pertussis (DTaP) en el esquema primario de vacunación pareciera, según los hallazgos del estudio citado, asociarse a una pérdida más rápida de la inmunidad, lo que condiciona que aumente el número de susceptibles. En Panamá, el esquema nacional público (MINSA - CSS) incluye el uso de vacuna pentavalente (DPwT + Hep. B + Hib) con componente pertussis de células completas, para la vacunación primaria.
